Duly certified copy of the charter of the legal entity or other document of title (stitched, numbered and sealed, if there is no seal - signatures are notarized)
Statute of the laboratory
Duly certified copy of the certificate of state registration of the legal entity (if any)
A duly certified copy of an extract from the unified state register, which must include, in particular, the following information: full and abbreviated name of the legal entity, location of the legal entity, types of activities, information on state registration, data on the legal entity's stay in bankruptcy proceedings, data on legal entities whose legal successor is the registered legal entity, data on legal entities-legal successors: full name and location
A duly certified copy of the certificate of registration of a value added tax payer or an extract from the register of value added tax payers
Survey questionnaire (f-08.17.15)
Laboratory passport (f-08.17.19)
Properly executed draft "Scope of Accreditation" (f-08.17.17) (sewn, numbered, signed, dated and stamped by the applicant)
Documentation of the management system in accordance with DSTU ISO/IEC 17025:2017 (list of documentation, full set of procedures and others)
Data on the location of the applicant's premises in the city (plan/sheet/copy of city plan)
Plan of overhead line premises and adjacent premises with equipment location
Copies of diplomas or other documents confirming the qualification of personnel in accordance with Form 6 of the passport of the Testing Laboratory
Copies of original test reports or sampling reports for each of the activities
